By signing up for Weight & Wellness Online, you will have limited access to our Electronic Health Records system via a secure patient portal. We are allowing all patients who sign up to request a medication refill, request lab test results, view their health summary (includes medications, allergies, medical history and immunizations) and ability to email the staff with health questions or concerns.

 

Please print and sign the Weight & Wellness Patient Portal Authorization Form and mail to our office or bring it in on your next visit.  Once your login has been activated by our office, you can click on "W&W Online" at the top, right-hand side of the menu bar. This will allow you to login and take advantage of all of these great features!
